Pool Cards

If you have lost your card, need an additional card, or are new to Oak Creek.

In order to obtain a card you will need these three completed items:

1. We will need a legible scan or picture of your Florida Driver’s License or Florida Identification Card with your Oak Creek address on your ID – no other document will be accepted.
    
2. A completed legible copy of the Pool Policy and Usage Guidelines signed and listing all members of the family.
    
3. Pool cards are $15.00 each and can only be paid via check made out to Oak Creek CDD.
